Traumatic Asphyxia

Abstract
In 1837, Ollivier found at autopsy of persons who had been trampled by crowds in Paris, craniocervical cyanosis, subcon junctival hemorrhage and Cerebral vascular engorgement. He applied the term "Masque ecchymotic¡±. Similiar reports subsequently appeared by many authors up to this time. But pathogenesis of the traumatic asphyxia syndrome remains obscure. It appears related to a voluntary or involuntary abdominal muscular contraction against a closed glottis after deep inspiration. Whether increased venous pressure in the head and neck is related to functional obstruction of the. superior vena cava or to retrograde ejection of venous blood from the thoracic cavity is unclear.
Recently we had experienced 15 cases of traumatic asphyxia in Dept. of General Surgery, National Seoul Hospital, that was developed by slipping down-from upstairs and trampling by crowds in majority, except two cases which was compression injuries by truck and cement block each other. And also : it is not our purpose to study the pathogenesis of the traumatic asphyxia nor specific treatment, but rather, to analysis, the clinical symptoms and signs, response to the, conservative treatment and diagnostic method:
And so we obtained the results: as follows
1) Traumatic asphyxia is a syndrome showing petechiae and purplish discoloration of skin of face, neck and upper part of the chest.
2) Mainly it develops by continuously severe compression on the chest and upper portion of the abdomen.
3) All 15 cases we experienced showed subconjunctival hemorrhage, ecchymosis or petechiae of face, neck and chest, edema of eyelid, face and neck, and flushing of face.
4) All cases were treated conservatively, but it should be emphasized that injury of the lung parenchyma or rib fracture are well checked.
5) All cases showed good results for the conservative treatment although there was one case of complication without mortality.
